Home management of anticoagulation

Abstract

The present review addresses current findings regarding home management of anticoagulation. The literature indicates that, after structured training, motivated patients with sufficient manual skills and eyesight are able to manage their own longterm, oral anticoagulation therapy. A stable level of anticoagulation can be achieved by weekly checks using a highly accurate, user-friendly coagulation monitor, thus reducing considerably the complication rate in comparison with conventional check ups by the local general practitioner or a specialist. Patient self-management not only saves costs, but it also increases patients' quality of life.
