Atherosclerosis is one of the main causes of cardiovascular disease and in turn lipid disorders lie in the root of atherosclerosis. A lot of efforts have been made to fight against lipid disorders and thus heart disease through dietary and pharmacological interventions. In this regard, statins have been a groundbreaking advance to reduce mortality and adverse events mainly by LDL-cholesterol lowering. But now, we may wonder which way to go beyond LDL and statins? The answer in this issue of European Heart Journal Supplements is HDL and nicotinic acid. Nicotinic acid has been traditionally hindered by untoward side effects like flushing and liver enzyme elevation. A prolonged release formulation (Niaspan®) appears as an equally effective, better tolerated alternative.
