Heart failure is a common and often severe clinical condition. Acute decompensations are currently regarded as specific situations for which more effective and safe treatments are needed. Nesiritide is a recombinantly synthesized exact copy of human B-type natriuretic peptide (hBNP). The utility of exogenous BNP is explained in this supplement by leading figures in the field. The rationale behind use of nesiritide emphasises the relevance of the heart as an endocrine organ. Acute heart failure syndromes have been studied in large registries like the Acute Decompensated Heart Failure National Registry (ADHERE) and Epidémiologie Francaise de l'Insuffisance Cardiaque Aiguë (EFICA).
